Submit an Application That Satisfies Approval Requirements

 

The facility must meet the following approval requirements:

  • It must be located in the US.
  • It must identify what locations within the facility or facility campus will apply for this recognition.
  • Completion of an application for approval to include:
    • Mission statement that supports Dementia Capable Care tenets.
    • Philosophy statement related to serving those with Alzheimer’s and related dementias and which supports Dementia Capable Care tenets.
    • Best practice examples to support implementation of Dementia Capable Care.
    • Reason(s) for seeking the Dementia Care Specialists Distinguished Provider recognition.
    • Identification of key personnel who have significant responsibility in supporting the Dementia Capable Care training principles including:
      • The Dementia Capable Care Authorized Instructor serving the facility (primary trainer).
      • The DCCT and the DCCCP who are serving the facility (must be at least a part-time employee/contractor servicing the facility) who has earned recognition as an individual Dementia Capable Care Therapist or Care Partner and who can fulfill the role as internal mentor as needed (may also be the same person as the Authorized Instructor).
      • At least one OT and one PT and/or SLP serving the facility who have completed the Dementia Capable Care: Foundation and Dementia Capable Care: Dementia Therapy Applications courses from Dementia Care Specialists.
      • The Executive Director/Administrator, DON, Director of Activities, and Dementia Unit/Program Director (if one exists) named with a summary of their role in providing and supporting successful Dementia Capable Care.
    • Definition of the role of therapy in providing and supporting Dementia Capable Care within the facility.
    • Definition of the purpose of the ADL and activity program in providing and supporting Dementia Capable Care within the facility.
    • Description of the facility process for (a) meeting the initial dementia training standards; (b) maintaining and tracking training records including identification of the responsible party(ies); (c) meeting the ongoing annual education standards; (d) submitting success stories, outcomes, surveys, and possible attendance/presentation at the Dementia Care Specialists leadership conference.

Important Notes About the Application Process

  • Typically the Dementia Care Specialists Distinguished Provider application and approval process takes approximately 4 to 12 weeks, providing the aspirant Dementia Care Specialists Distinguished Provider facility fulfills their responsibilities promptly.
  • The facility must submit a nonrefundable fee of $400 with the application.
