CH-3SM Skills for Clinical Holding addresses the complex professional issues associated with delivering essential care and treatment to people who are unable or unwilling to comply with nursing and medical treatment due to a lack of mental capacity.
These situations can present a significant risk to the safety of the individual or staff, or may lead to a significant deterioration of the individual’s welfare, health, or medical condition.
CH-3SM Skills for Clinical Holding is appropriate for all nursing and medical staff and covers clinical issues relevant to all acute departments and clinical specialties. This training programme helps medical and clinical professionals:

CH-3SM Skills for Clinical Holding is a two-day direct delivery training programme. It includes four hours of practice application in a range of simulated clinical environments, including treatment rooms and surgeries, as well as on clinical equipment such as beds, trolleys, wheelchairs, and hoists. Participants must complete a six-hour annual re-certification programme in order to maintain their competency.
Programme Aims
When individuals who lack capacity—as a result of impaired cognitive ability or an underlying medical condition—are unable to comply with a specific procedure, critical decision-making is vital. Staff involved in the essential care and treatment must act in the best interests of the patient in order to maintain the patient’s health, safety, and welfare. Stopping a procedure could negatively affect a person’s welfare, health, or medical condition, or could cause injury.
When treatment cannot be discontinued, staff must make decisions by weighing factors such as risks, legalities, and professional guidelines. CH-3SM Skills for Clinical Holding helps staff make these decisions by providing a range of non-verbal, verbal, supportive and physical interventions that are unique to these situations.
Upon successful completion of CH-3SM Skills for Clinical Holding, participants will be able to:
- Describe reasoning for decisions made to restrict a person’s movement that can be evidenced against legal and professional benchmarks for risk reduction.
- Explain the risks associated with the use of restrictive holds or physical restraints.
- Demonstrate verbal and non-verbal de-escalation strategies.
- Demonstrate use of holding skills consistent with a set of physiological principles that may be used in varying clinical environments to restrict movement and minimise risks.
- Demonstrate how to disengage from various holds using a consistent set of physiological principles.
Instructor Certification
The five-day Instructor Certification Programme allows organisations access to the entire CH-3SM curriculum and materials for ongoing staff training. This programme is delivered at our training suite in Newcastle-under-Lyme, Staffordshire. Once certified, Instructors are required to complete a 12-hour annual re-certification and licensing programme.
Closed/Commissioned Programmes
