That's why we've developed a set of implementation workshops to help your leadership team make person-centered, stage-specific care a reality. Learn more about each of our four workshops below.
the Dementia Capable Care: Foundation Course
is a prerequisite to experiencing these workshops.
CORE is exactly what it sounds like—the building blocks for delivering person-centered, dementia stage appropriate care. It's where we begin. During our CORE module, teams are introduced to the person-centered and cognitive assessment suite. These tools and templates will help identify preferences, routines, and interests, as well as cognitive level and remaining abilities. Using this assessment information, your team will create person-centered care plans for each individual that will improve their quality of life, engagement, health and well-being.
Activities are the heart and soul of a quality memory care program. In this workshop, your leadership team will learn to apply the results of the person centered, cognitive level assessments with CORE to create individualized activity plans. You'll learn how to use our activity assessments and our Dementia Care Specialist Activity Tool Kit to create calendars and implement programming that is customized to meet individual interests and preferences while supporting cognitive needs.
ADLs & Daily Routine Module
Abilities based care focuses on remaining abilities at every stage of dementia. This includes facilitating abilities to peform activities of daily living (ADLs) as independently as possible. In this workshop, your team will learn to apply the results of the person centered, cognitive level assessments with CORE to create individualized ADL plans. You'll use our person-centered cognitive assessment tools powered by our abilities-based approach to optimize and maintain function and prevent avoidable ADL decline. In addition, we provide tools to inform staff planning based on level of care needs.
Behavior Management Program Module
Individuals with dementia rely on other forms of communication, such as facial expressions, gestures, body language and behavioral expression to communicate their wants and needs. In this workshop your team will learn to apply the results of the person centered, cognitive level assessments with CORE to create individualized behavior management plans. We provide your staff with a systematic way to recognize the triggers and patterns causing the disruptive behaviors which often result in the overuse of psychotropic drugs, altercations, and injuries. With these tools your team can deliver a nonpharmacologic care approach for behavior management that is person-centered and dementia stage informed. This approach will promote safety, security and well-being of all the individuals in care.
